Indiana Nurse Practitioner Collaboration 2026: Reduced Authority & Collaborative Practice Agreements

This guide explains Indiana’s NP collaboration requirements, what reduced authority means in practice, and how nurse practitioners can stay compliant while maintaining significant clinical autonomy.

    Indiana NP Practice Laws

Everything Indiana nurse practitioners need to know about collaborative practice agreements, prescriptive authority, compliance requirements, market rates, and finding a collaborating physician. Updated for 2026.

Indiana doesn't give nurse practitioners an independent path — at least not for prescribing.

Indiana is a reduced (collaborative) practice state. If you want to prescribe medications — whether you're running a brick-and-mortar clinic or a telehealth practice — you need a written Collaborative Practice Agreement (CPA) with a licensed practitioner and separate prescriptive authority granted by the Indiana State Board of Nursing before you write your first prescription. This framework is governed by Indiana Code Title 25, Article 23 (IC 25-23).

That's not changing in 2026. For Indiana nurse practitioners trying to launch, grow, or keep a practice compliant, that means understanding the CPA rules, prescriptive authority limits, controlled-substance requirements, physician availability expectations, geographic and telemedicine flexibility, typical collaboration costs, how to find the right physician, and what ongoing compliance with the Board requires.

For most Indiana NPs, the challenge isn't understanding the rule. It's structuring the agreement correctly, finding a physician who actually fits their practice, and doing it without overpaying, disrupting operations, or creating licensing and legal risk. This guide covers all of it.

Hello, my name is Philip and I graduated from Florida International University for medical school and the University of South Florida for psychiatry residency. I have experience working at the outpatient PTSD clinic at the Tampa VA as well as inpatient at the largest baker act receiving facility in the Tampa Bay area called Gracepoint. I also have experience covering the psychiatry ER as well as the inpatient units at the Tampa VA. I have also worked for different outpatient psychiatric telemedicine platforms online. Throughout my training and work, I have gained much experience in treating mood and anxiety disorders, trauma-related disorders, personality disorders, eating disorders, dissociative disorders, psychotic disorders, bipolar disorder, as well as substance use disorders. More recently I have gained a passion for working with NPs which is a major need in the field of mental health. Countless patients have suffered through incredibly long wait times in order to be seen by a mental health provider. As awareness for mental health increases around the nation, the need for more immediate treatment has never been greater and I fully believe that NPs have a very large role to play here. I have over 15 different NPs that I collaborate with at this time. I pride myself in being easy to work with and provide each NP with my direct cell and they can reach out at anytime. Conversations typically start with a quick text and we can schedule a phone call or video meeting from there if needed. The initial meeting is used for introductions and to assess each other's prescribing methods and approach to certain cases. I understand that supervision may not be needed for standard non-complicated cases and respect each NPs freedom to run their practice as they see fit. However, I have much experience in treating complicated cases so I urge NPs to never hesitate to reach out at any time, even if it is for a quick question. ADD $100 FOR OHIO AND ALABAMA. WHAT INDIANA REQUIRES FOR NURSE PRACTITIONER LICENSURE AT A GLANCE

    Requirement

    Indiana Rule

    Practice Authority

Reduced — collaborative practice required for prescribing

    Collaborative Practice Agreement (CPA)

✅ Required — written, signed by both parties

    Board Filing / Retention

✅ Maintained by the NP; audited by the Board every two years

    Prescriptive Authority (APRN-RX)

✅ Separate application to the Indiana State Board of Nursing

    Controlled Substances

✅ Schedules II–V permitted with Indiana CSR + federal DEA registration

    Collaborator Qualifications

    ✅ Indiana-licensed practitioner (typically an M.D. or D.O.) in a compatible scope

    Physician Ratio Cap

    ✅ No statutory numerical limit

    Geographic Proximity Requirement

✅ None statewide — physician availability must be maintained

    Telemedicine Collaboration

✅ Permitted — same standard of care as in-person

THE COST OF GETTING THIS WRONG

Before we get into the requirements, let's be direct about what's at stake.

Practicing without a valid CPA — or with an agreement that doesn't meet Indiana State Board of Nursing standards — isn't a paperwork problem. It's a licensing problem. The Board has independent enforcement authority and can investigate, sanction, and suspend your ability to practice. Sanctions range from a letter of reprimand and probation to suspension, revocation (with no reapplication for seven years), and fines up to $1,000 per violation.

The most common compliance failures Indiana NPs face:

    • Missing or invalid CPA — practicing before a valid written agreement with a licensed practitioner is in place

    • Lapsed or outdated CPA — practice scope or collaborator changed but the agreement wasn't updated

    • Prescribing before authority is granted — issuing prescriptions before the Board grants separate prescriptive authority

    • Missing CSR/DEA registration — prescribing controlled substances without both an Indiana Controlled Substance Registration and a federal DEA number

    • INSPECT non-compliance — failing to query Indiana's prescription monitoring program (INSPECT) as required before controlled-substance prescribing

    • Scope mismatch — prescribing outside the NP's scope or the collaborating practitioner's scope

Any one of these can trigger a Board investigation. Because Indiana audits collaborative practice agreements every two years, violations often surface even without a patient complaint. A license suspension doesn't just pause your practice — it can end it.

THE COLLABORATIVE PRACTICE AGREEMENT (CPA) AND PRESCRIPTIVE AUTHORITY: WHAT IT MUST INCLUDE

The CPA is the legal document that anchors your prescribing in Indiana. It is a written agreement between an advanced practice registered nurse and a licensed practitioner. It must be written, signed by both parties, and retained by the NP — available for inspection and produced if the Board selects you for its biennial audit.

A valid Indiana CPA must include:

    • Complete names, addresses, and license numbers of both the NP and the collaborating practitioner

    • The type of collaboration arrangement between the parties

    • Practice settings and locations covering every site where you'll practice under the agreement

    • Scope of delegated prescriptive authority, including authorized drug classes and any conditions, kept within both the NP's and the collaborator's scope of practice

    • Provisions for timely review of the NP's prescribing practices

    • Consultation, referral, and communication procedures, including how the collaborator is reached and coverage during absences

    • Signatures of both parties

One important nuance specific to Indiana: the statute uses the broader term "licensed practitioner" rather than "physician." The collaborator is almost always a physician (M.D. or D.O.), but the law excludes veterinarians, other APRNs, and physician assistants from serving in that role.

A second critical distinction: In Indiana, the NP does not file the agreement with the state at the outset the way some states require. Instead, the NP maintains the CPA and submits a separate application for prescriptive authority to the Indiana State Board of Nursing, where you pay the $50 prescriptive-authority fee separately from the broader nurse practitioner licensure application fee of about $100. That application must also provide verification of certification from a recognized national certification organization. National certification is required for licensure, and the American Nurses Credentialing Center is one such recognized body. To be eligible to sit for the certification exam, the NP must hold an accredited graduate degree in nursing and an active RN license. If audited, the NP must submit copies of all agreements in place during the prior licensure period, along with a sworn statement signed by both the NP and the primary collaborator confirming compliance. The NP carries the responsibility for keeping the agreement current.

The CPA should be reviewed and updated any time your clinical practice scope changes — new specialties, new locations, a new collaborating practitioner, or new prescribing needs.

CONTROLLED SUBSTANCES: WHAT INDIANA NPS CAN AND CANNOT PRESCRIBE

With a valid CPA and prescriptive authority, an Indiana NP may prescribe Schedule II through V controlled substances, as well as legend drugs, subject to the limitations defined in the agreement and applicable prescribing standards.

For any controlled substance prescribing, you also need:

    • An active federal DEA registration number (DEA Form 224 for new applicants; roughly $888 for the three-year cycle)

    • An Indiana Controlled Substance Registration (CSR) — a state-level registration separate from the DEA — obtained before prescribing any controlled substance

    • Compliance with INSPECT, Indiana's prescription monitoring program, including querying as required

    • Prescribing that stays within the drug classes and conditions authorized in your CPA and within both your scope and the collaborator's scope

To qualify for initial prescriptive authority, the NP must have completed a graduate-level pharmacology course of at least two semester hours within the five years before applying (with an alternative CE-plus-experience pathway if the course is older). The NP license and prescriptive authority both renew every two years in Indiana and expire on October 31 of odd-numbered years. Renewal also includes continuing education requirements: Indiana requires at least 30 CE hours in each renewal cycle, and maintaining prescriptive authority requires 30 CE hours every two years, including at least 8 hours in pharmacology (reduced requirements apply for authority held under 24 months).

Note on 2026 legislation: bills such as HB 1129 and HB 1116 have proposed removing the collaborative agreement requirement and expanding Schedule II authority (including for weight reduction/obesity). As of this update, these have not become law, so the collaborative framework remains in effect.

PHYSICIAN RATIO LIMITS AND GEOGRAPHIC FLEXIBILITY

    Collaboration Limits

Unlike some states, Indiana does not impose a specific statutory cap on the number of APRNs a single physician may collaborate with. The practical limit is that the collaboration must remain meaningful — the collaborator must be able to provide timely review of prescribing practices and genuine consultation. A collaborator spread across too many practices may not meet that standard, which becomes the NP's compliance risk during an audit.

    No Statewide Geographic Distance Requirement

Indiana does not impose a general mileage rule for the collaborating practitioner. Your collaborator can be located elsewhere in the state, provided they remain available for consultation and can perform the required timely review of your prescribing. This is a significant advantage for NPs in rural Indiana counties and those building practices across the state — though the arrangement works best when the collaborator's practice is comparable to your patient population and specialty focus. One exception to be aware of: at Critical Access Hospitals (common in rural Indiana), a physician must periodically review and sign inpatient records even when not physically present.

ONGOING COMPLIANCE WITH THE INDIANA STATE BOARD OF NURSING: CONTINUING EDUCATION REQUIREMENTS, DOCUMENTATION, REVIEW AND AVAILABILITY

Signing a CPA is the starting line, not the finish line. Indiana expects active, documented collaboration throughout the relationship — and because of the biennial audit, documentation is what protects you.

    Prescribing Review and Chart Documentation

Your CPA must include provisions for timely review of your prescribing practices. The collaborating physician must review 5% of charts in which medications are prescribed, which helps define oversight duties in practice. Maintain written records of:

    • Date of each review or consultation

    • Charts or prescriptions reviewed (by identifier, HIPAA-compliant)

    • Collaborator comments or recommendations

    • Any follow-up actions required

    The Biennial Audit

The Board audits collaborative practice agreements every two years. If selected, you must produce all agreements in force during the prior licensure period plus a sworn compliance statement signed by you and your primary collaborator. Indiana renewal also requires 30 CEUs, including at least 15 hours in pharmacology, so track that documentation as part of your compliance workflow and broader regulation recordkeeping. Failing to produce valid agreements can trigger disciplinary proceedings even absent any patient complaint — which is why keeping clean, current records is non-negotiable.

    Continuous Availability

Your collaborating practitioner must remain reachable for consultation during your practice hours — not physically present, but available by phone or electronic communication. This is a patient safety expectation, not just paperwork.

    Keeping the Agreement Current

Update the CPA whenever your scope, sites, collaborator, or prescribing needs change, and ensure the sworn statement and license numbers on file remain accurate.

    TELEMEDICINE COLLABORATIONS IN INDIANA

Indiana permits NPs to deliver care through telehealth under IC 25-1-9.5, and APRNs with prescriptive authority may prescribe medications during telehealth visits — including to patients they have not previously examined in person. The governing principle is straightforward: a practitioner providing telehealth services is held to the same standard of care as one providing in-person care. The same CPA and prescriptive authority requirements apply.

Key considerations for Indiana telehealth NPs:

    • Verify the patient's identity and location, and disclose your name and licensure at the encounter

    • Obtain informed consent for the telehealth visit and gather enough history to establish a diagnosis

    • Maintain a medical record just as you would for an in-person visit, using HIPAA-compliant resources and systems that support documentation and virtual workflows

    • Notify the patient's primary care provider when you issue a prescription (with consent, subject to exceptions)

    • Ensure INSPECT is queried for controlled-substance prescribing, and keep collaboration review and consultation working in the virtual setting

    • Conduct all collaborator communication on HIPAA-compliant systems

NPCollaborator's physician network includes physicians with active telemedicine experience across multiple states, many of whom are already set up for virtual collaboration and prescribing-review workflows and can support Indiana NPs in maintaining compliant virtual prescribing-review processes.

WHAT DOES A COLLABORATING PHYSICIAN COST IN INDIANA?

This is the question every Indiana NP asks — and the answer varies more than most people expect.

The general market range in Indiana falls between $500 and $1,200 per month, with an average around $850/month. Several factors drive that variation:

    • Specialty complexity — Mental health and aesthetics command higher rates due to controlled substance oversight and liability

    • Controlled substance authority — Broader Schedule II–V delegation increases costs

    • Physician experience and availability — Experienced physicians with open capacity are in higher demand

    • How you find them — Personal networking and cold outreach almost always cost more than a matching platform

The difference between finding a physician through cold outreach versus a matching platform isn't just convenience. It's often $200 to $400 per month, every month, indefinitely. Over a year, that's $2,400 to $4,800 in unnecessary overhead.

HOW TO FIND A COLLABORATING PHYSICIAN IN INDIANA

There are three routes — and they're not equal.

    • Employer-assigned: Simple, but you have no control over physician fit, engagement, or what happens if they leave. Doesn't apply if you're building your own practice.

    • Personal networking and cold outreach: Can work, but slow, unpredictable, and typically above-market pricing. Weeks of searching with no guarantee of a specialty match — a particular problem in rural Indiana counties where few physicians practice locally, which is precisely where collaboration bottlenecks are most acute.

    • Matching platform: Pre-vetted physicians, transparent pricing, faster matching, and compliance infrastructure built in — including help preparing audit-ready documentation. For most independent NPs, this is the fastest and most reliable path.

Bureau of Labor Statistics labor statistics project 45.8% NP employment growth in Indiana from 2022 to 2032, one reason demand stays high as more healthcare professionals enter or expand practice to meet broader healthcare needs and build long-term careers.

WHAT TO LOOK FOR IN A COLLABORATING PHYSICIAN (BEYOND THE CREDENTIAL CHECK)

Finding a physician who is licensed and available is the floor, not the ceiling.

The right collaborating physician understands your specialty, communicates reliably, and treats the relationship as a genuine clinical partnership built on shared responsibility for patient care and patient outcomes — not a monthly transaction. If you've had a bad experience with a collaborating physician before, you already know the difference.

    Before signing any agreement, ask:

    • Do they hold an active Indiana license? Non-negotiable for your CPA to be valid, and they should be a licensed practitioner in good standing (not a veterinarian, APRN, or PA, which the statute excludes).

    • Does their scope align with your practice? Your prescribing is limited to both your scope and theirs — a family medicine physician on a psychiatric practice is a different dynamic than a psychiatrist doing the same, so their expertise should match your specialty and patient population.

    • What's their availability for real-time consultation and prescribing review? Indiana requires timely review of your prescribing, so make sure they can actually deliver it, especially when responsiveness is critical to improving access in rural Indiana or complex care settings.

    • Will they sign the sworn compliance statement at audit time? Confirm they'll stand behind the agreement if the Board selects you for its biennial audit.

    • What's their communication style? Regular review and consultation are expected, so you need someone you can actually work with.

NPCollaborator's matching process is built around these questions — not just credential verification, but clinical and operational fit. Every physician in the marketplace has been vetted for specialty alignment, availability, and collaborative track record.

    FREQUENTLY ASKED QUESTIONS

How much does a collaborating physician cost in Indiana?
Market rates typically range from $500 to $1,200 per month depending on specialty and prescribing needs. NPCollaborator's marketplace starts at $449/month with malpractice coverage and legal documents included — below the Indiana market average.

Do I need a collaborating physician in Indiana?
Yes. Indiana is a reduced practice state. To prescribe, you must have a written Collaborative Practice Agreement with a licensed practitioner and separate prescriptive authority from the Indiana State Board of Nursing, underscoring the importance of maintaining professional compliance with state requirements. (Certified registered nurse anesthetists are treated separately under the statute.)

What are the legal requirements for an Indiana CPA?
A valid CPA must be written and signed by both parties, and include the names, addresses, and license numbers of the NP and collaborator, the type of collaboration arrangement, and provisions for timely review of the NP's prescribing. It must be retained by the NP and produced if the Board audits you.

Does the NP file the CPA with the state?
Not at the outset. The NP maintains the agreement and applies separately for prescriptive authority, and must keep documentation available for verification during Board processes or audits. The Board then audits collaborative practice agreements every two years, at which point the NP submits copies plus a sworn compliance statement.

Does my collaborator have to be a physician?
The statute says "licensed practitioner," and in practice that's almost always a physician (M.D. or D.O.). The law specifically excludes veterinarians, other APRNs, and physician assistants from serving as the collaborator.

Can I prescribe controlled substances in Indiana?
Yes, Schedules II–V, with a valid CPA and prescriptive authority, an active federal DEA registration, and an Indiana Controlled Substance Registration (CSR), and in compliance with INSPECT.

Does Indiana cap how many NPs one physician can collaborate with?
No. Indiana has no statutory numerical ratio cap. The practical limit is that the collaboration must remain meaningful and the collaborator must be able to review your prescribing in a timely way.
